If you are a full time employee, you will be eligible for benefits on the first day of the month following 60 days of full time employment.
Medical: United Healthcare
Cost: Employee Only: $45.19 per pay period. Employee & Spouse: $94.90 per pay period. Employee & Child/ren: $85.86. Family: $131.04. Medical co pays are $25 for primary physician and $50 for specialists; co-insurance is 80%
Dental: Principal Dental provides 100% preventative services and 80% basic services and 50% major services.
Employee only: $10.12 per pay period. Family: $27.79 per pay period.
Vision:EyeMed
Employee only: $2.98 per pay period. Employee & Spouse: $5.66 per pay period. Employee & child(ren): $5.96 per pay period. Family: $8.76 per pay period.
Life: OHB’s life insurance plan provides coverage equal to 2 times your annual earnings rounded to the nearest $1,000 with a $50,000 maximum. OHB pays the cost of the premium.
Accidental Death and Dismemberment: AD&D benefits are provided in coverages listed in the plan booklet. OHB pays the cost of the premium.
Long Term Disability (LTD): After a 90 day elimination period, eligible employees may qualify for LTD benefits paid at 60% of salary. Certain maximums and guidelines do apply. OHB pays the cost of the premium.
Lifestyle Protection Term Life Insurance: OHB provides optional life insurance coverage for those wishing to purchase additional life insurance on yourself, your spouse and/or your dependent children. You choose the amount of coverage and pay for it through payroll deduction.
Cancer & Hospital Intensive Care Insurance: Please see booklet details for coverage and cost of this plan. You pay premiums through payroll deduction.
Long Term Care: Nursing home, home and community based care, respite care (relief for a family member who provides unpaid care). You pay premiums through payroll deduction.
Flexible Spending Account: You may set aside pre-tax dollars for medical and dental and/or dependent care expenses and use it to pay certain kinds of out-of-pocket expenses. This means that you will pay less tax and have more spendable income. You also pay for your family medical premium with pre-tax dollars.
401k Retirement Plan: OHB has established a 401k Retirement Plan that permits you to contribute to your retirement income while taking advantage of reduced federal and state income taxes and convenient payroll deductions. If you are 21 or older and are regularly scheduled to work more than 20 hours per week, you are eligible to participate at the beginning of the calendar quarter following your 90th day of employment. OHB will contribute $.50 to the plan for every $1.00 you contribute up to 6% of your pay. This plan will accept rollovers from other qualified plans.
