Materials Needed to Initiate Application Process

  1. A completed Omaha Home for Boys application.
  2. Social History
  3. Psychological evaluation (completed within last 3 years)
  4. School information (the following must be included):
    • Attendance record.
    • Health record.
    • Behavioral reports (referrals to the office).
    • Permanent record card (transcript).
    • Individual Education Plans (if special education services are being provided).
  5. All other evaluations, progress reports and discharge summaries. Possible sources of this information would be counselor/therapists, hospitals or mental health centers, court/probation officers, social service agencies, other out-of-home placements, etc.
  6. Based on individual cases, the Omaha Home for Boys may require additional assessments be completed (chemical dependency, psychiatric, ADHD, etc.).

ASSESSMENT:
A psychological assessment is conducted by an OHB licensed psychologist. The following types of assessments are utilized:

  1. Intellectual
  2. Academic
  3. Behavioral and social skills (SBCL, Eyeberg, & SSRS)
  4. Reynolds Depression

ACCEPTANCE/REJECTION OF APPLICATION:
Once the tour and interview have been completed, the application is presented to the Omaha Home for Boys’ Admissions Committee. The Admissions Committee meets on a weekly basis to formally review all applications before referral to the House Committee for final approval.

ADDITIONAL PLACEMENT DOCUMENTATION:
The following must be provided prior to placement:

  1. Certified birth certificate.
  2. Social security card.
  3. Physical examination (no older than 30 days).
  4. Immunization records.
  5. Copy of certificates: i.e. death, divorce, marriage, (if applicable).
  6. Court order (if applicable).
  7. Interstate Compact (for all out-of-state applicants).
  8. Detailed family medical history.

TIME FRAME:

  1. Within seven days of receiving all necessary paperwork (i.e. application, social history, school records, evaluations, etc), the admissions coordinator will set up a tour and interview for youth who meet all inclusion and exclusion criteria.
  2. Within seven days of the tour and interview, the admissions coordinator will compile the information into a report to be presented to the Admissions Committee for review. Approval for admittance or rejections occurs at this time. The Admissions Committee meets once each week.
  3. For youth who have been approved for admittance, parents/guardians will be notified within two business days and admitted within seven business days.
  4. For youth who have been rejected for admittance, parents/guardians will be notified within five business days.
  5. For youth who have been rejected for admittance, parents/guardians have the right to file an appeal to have their case heard by the Admissions Committee again.

PLACEMENT PROCESS:

  1. Placement date is established with the family and/or placing agency (the remaining required materials are once again requested and are necessary by the date of intake).
  2. The Admissions and Family Service specialist documents the completion of the outstanding paperwork on the day of intake, which includes a signed placement agreement from the legal guardian and/or placing agency (no placement takes place without this signed agreement).
  3. Assigned OHB case manager and House Parent meet with family and youth.
  4. Case manager provides, in writing, a list of the administration’s and house parent’s phone numbers, e-mail addresses and mailing addresses.
